The pistol belts have 5 holes to insert the belt tongue through.  The size measurement of the belt is taken from the fold of the belt as it attaches to the belt buckle to the MIDDLE hole in the belt. For example, the 40" size belt, measures 40" from the fold of the belt as it attaches to the belt buckle to the MIDDLE hole that the buckle tongue would be inserted.
